Getting ready for the next steps

Moving between childhood and adulthood can be exciting but also challenging. This is especially true for children and young people with Special Educational Needs and Disabilities (SEND). The transition may take longer and need more preparation.

At Newcastle City Council, we work with families, schools, and services to support them on this journey. Every young person is different, so planning should start early and be based on their needs and hopes.

With the right support, we can increase their skills, confidence, and independence for life after school.

Take a look at age appropriate outcomes that can prepare a child or young person for adulthood.

SEND transition guide

This is a detailed guide for parents and carers. It contains all the ways a child with SEND can be supported as they prepare for adulthood.
